Whether you are a Service Coordinator, a Managed Care Coordinator or a Private Service Coordinator you are very important to individuals with IDD or neurodiversity and their families. For person’s with disabilities and their families, navigating the system of available services is often a confusing task. Assisting individuals and families in gaining access to necessary services and supports appropriate to the needs of the person will help achieve quality of life with outcomes that lead to successful inclusion into their community. This includes living in the housing setting of their choice, with the supports they need. As you guide individuals through person-centered planning you are helping them achieve their goals and realize their dreams.
