My Independence Action Plan (MIAP)

Community for Permanent Supported Housing (CPSH), a Texas nonprofit providing independence skills resources and housing options, has been selected by the Texas Workforce Commission as a vendor to provide Pre-Employment Transition Services for students with disabilities 14 to 22 years of age in public and private schools. Services focus on Workplace Readiness, Self Advocacy and Housing Options.

According to CPSH President Robin LeoGrande,  “with this opportunity we are able to provide pre-employment planning as a key component of person-centered and family-centered independent living and further support students with disabilities. We look forward to working with students and their support networks.”

CPSH will focus on assisting students in first, making plans for their futures in the areas of community living, work preparation and support services and second, identifying short and long term actions, with the participation of their support networks, to implement their plans. The plans and actions inform the student’s IEP, Person-Centered Plan, Supported Employment Services Plan, other service plans, as well as their and their family’s financial plans and legal documents.

The course is offered in two parts.

  • Part One - MIP (My Independence Plan): Available in person for individual family, virtually (individual or group session) or in school (group session). A member of each student’s support network is required to attend.

  • Part Two - MAP (My Action Plan): An individualized (private) session available virtually or in person. A member of each student’s support network is required to attend.

  • If you are not currently a Texas Workforce Solutions customer:

    • Contact your local Vocational Rehabilitation office: https://webp.twc.state.tx.us/services/VRLookup/ and ask for Pre-Employment Transition Services.

    • Complete the following forms:

      • VR1517-2, Authorization to Release Confidential Customer Records and Information

      • VR1820, Request to Receive Pre-Employment Transition Services
